Account, Access and Data Deletion

1. WHO MAY SUBMIT A REQUEST

Patients may request closure of a Prime Diagnostics app or portal account and deletion or anonymisation of eligible account information.

The public patient request form is available without signing in at https://results.primerad.co.za/data-deletion-request.php. Signed-in patients may also start a request from the mobile application.

Healthcare practitioners may request revocation of access and deletion or anonymisation of eligible practitioner account information by contacting admin@primerad.co.za. A dedicated practitioner request form will be added to the account-management workflow.

2. PATIENT REQUESTS

An approved patient request may include disabling the portal account, revoking active mobile sessions and deleting or anonymising eligible registration, contact and portal information.

Radiology images, reports and related clinical records are not automatically deleted when an app account is closed. They may need to remain in the healthcare record for continuity of care and legal, regulatory or professional recordkeeping duties.

3. PRACTITIONER REQUESTS

A practitioner request may include revoking mobile and portal access, removing or disabling associated access in connected clinical systems, revoking active mobile sessions and deleting or anonymising eligible practitioner-registration information.

Practitioner requests are completed only after an authorised administrator confirms that related clinical-system access has been removed or disabled.

4. INFORMATION THAT MAY BE RETAINED

Prime Diagnostics may retain medical records, legal or regulatory records, security evidence, audit information, fraud-prevention records and minimal proof that a deletion request was received and completed where retention is required or reasonably necessary.

Where possible, retained deletion-request records are reduced, scrubbed or anonymised after completion.

5. VERIFICATION

Prime Diagnostics may contact the requester and require reasonable proof of identity, professional authority or control of the registered email address before completing a request. This protects medical and identity information from fraudulent deletion.

6. REVIEW AND COMPLETION

Requests may be marked pending, in review, completed or rejected. A request may be rejected where identity or authority cannot be verified, the request is fraudulent or abusive, or the information must lawfully be retained.

7. SUBMIT A REQUEST

Patients may use https://results.primerad.co.za/data-deletion-request.php. Healthcare practitioners may send an access-revocation and account-data request to admin@primerad.co.za.
